Taking part in a PADI diving course with us?
The following forms relate to your PADI courses that you will be required to fill in before commencing any PADI course with Silent Bubbles Scuba Diving. When you book with us we will give you the correct forms to fill in but to save time you can fill in and print the forms related to your course.
Please complete, print out and sign all appropriate PADI course forms and bring all the approprite forms to the first day of your PADI course. (Click on each item to download it). Adobe Reader is required to view and fill out the forms, click here if you havent got it on your PC for a free download.
The PADI medical statement is NOT required to be signed by your doctor UNLESS you answer ‘YES’ to any of the questions. If you have answered ‘YES’ to ANY question (regardless of how long ago the condition relates to), you must be examined by your doctor before participating in diver training.
